PT Handbook Conversation: Back Pain Should Fear You

Let’s continue one with the theme of last weeks newsletter….If you’ve been told that back pain means something is “out of place” OR “out of alignment” OR that you should avoid movement, don’t listen to them. The latest research in pain science shows that pain doesn’t always equal damage. Moreover, many cases of back pain aren’t due to structural issues but rather a combination of stress, movement habits, and sensitivity in the nervous system. This is the biopsychosocial approach to pain—where your thoughts, emotions, and lifestyle all play a role in how you experience discomfort. Avoiding movement out of fear can actually make your pain worse by increasing stiffness, weakening muscles, and making your brain more sensitive to pain signals. I’M ALSO NOT SAYING THE PAIN IS ALL IN YOUR HEAD, everything works together in the human body, mental and physical.
Instead of fearing your pain, learn to move with confidence. Gradual exposure to movement, strength training, and addressing lifestyle factors like sleep and stress can retrain your nervous system to feel safe again. Your back is strong, adaptable, and resilient—one flare-up doesn’t mean permanent damage. A skilled doctor of physical therapy can help you find safe ways to move, calm the nervous system, and regain control over your pain.
